1,000 people sent me their addiction treatment stories. Here’s what I learned.

Since September, I’ve been asking Vox readers to share their personal experiences with drug addiction treatment as part of ourRehab Racket initiative. In less than four months, we’ve received a large number of answers – more than 1,000 so far. These range from devastating anecdotes of overdoses and early deaths to more upbeat accounts of rehabilitation. People from all across the United States, including every state and Washington, D.C.; Canada; the United Kingdom; Germany; Switzerland; South Africa; and Australia have gathered to take part in the event.

  1. One common thread emerges from all of these stories: Addiction treatment in the United States is expensive, difficult to navigate, and far too frequently fails to deliver results.

I wanted to take a time to reflect on what I’ve learnt so far this year, especially as the end of the year approaches. Here are four things I’ve learned from the project so far, as well as my plans for the future.

1) Poor insurance coverage leads to high addiction treatment costs

Almost everyone I spoke with had at least some difficulty in having addiction treatment covered, either because they did not have insurance or because their insurer refused to fund the treatment they need. All of this contributes to understanding why drug addiction treatment in the United States may be so expensive: Insurance is the system that is supposed to protect Americans from massive health-care bills, but it is not always effective in this regard when it comes to addiction treatment.

2) We know disturbingly little about addiction treatment in America

When I’ve been reporting on addiction treatment in the United States and speaking with specialists on the evidence for addiction therapy, there’s one response I’ve grown accustomed to hearing: “I don’t know.” When I inquired about the effectiveness of specific addiction therapies, I received this response frequently. Many facilities, for example, are increasingly utilizing equine therapy, which allows patients to form bonds with horses. However, there has been little investigation into whether this is effective.

It is not that these procedures have been shown to be ineffective.

This is a very unusual occurrence in the context of what is intended to be health care: Normally, therapies are based on scientific data.

However, in the field of addiction therapy, a lack of proof for success is a common occurrence.

However, while some of these facilities promise to do so, they often rely on follow-up questionnaires that are rife with bias and inaccuracies; for example, a patient might say that he hasn’t taken drugs, and the survey taker will make no attempt to really check that claim; This is not only a failure of the industry, but also a failure of the law and the regulatory system in general.

It’s just that there hasn’t been much investigation into it.

For the time being, however, patients are left in the dark regarding whether or not a treatment center is truly following the evidence.

3) We don’t make good use of what we do know about addiction treatment

There are, however, some things we know about addiction therapy that we should share with you. Unfortunately, we do not make effective use of the data we have at our disposal. For example, research has shown that drugs for opioid addiction — methadone, buprenorphine, and naltrexone — are quite helpful in treating the condition: According to studies, they reduce the death rate among opioid addiction patients by half or more and help individuals stay in treatment longer than non-pharmacological alternatives.

  • Data from the Federal Substance Abuse and Mental Health Services Administration (SAMHSA) shows that just 42 percent of the approximately 15,000 institutions tracked by the agency provide any form of medicine for opioid addiction.

However, it has garnered little attention from policymakers in the United States. To put it simply, we won’t go very far in tackling the opioid problem or addiction in general until and until we start using what we already know to be effective.

4) Stigma is a huge issue

When I’m writing about addiction or anything linked to it, there’s one reason I can count on for almost any problem: stigma. Indeed, it is the common thread that runs through all of the teachings on this list. This has been with me ever since I visited Vermont in 2017 to examine how the state was able to build up its substance abuse treatment infrastructure. When I asked authorities about the obstacles to establishing the new system of care, I expected to hear a lot about money. Instead, I heard a lot about people (a common problem in policymaking).

  • It’s nearly unfathomable, to be honest.” That is how these problems continue to exist: Until a sufficient number of individuals overcome their negative attitudes regarding addiction, they will not give the problem the attention it deserves to be resolved.

One piece of good news is that a large number of people are prepared to fight this social stigma. This is demonstrated by the fact that Vox got more than 1,000 replies to their poll, with the majority of respondents providing their complete names and personal information.

What’s next?

The Rehab Racket project is expected to endure until at least 2020, and the comments my editors and I received to our survey have provided us with various leads and ideas that we want to pursue in the future. One subject that I’ll be covering in the near future is the domination of the 12 steps. Some people find this treatment strategy, which is based on the principles of Alcoholics Anonymous, to be beneficial, and it has a strong empirical approach when it comes to treating alcohol addiction.

Finally, I’d want to focus on some of the more particular concerns that arise in the treatment of addiction.

One of the most absurd aspects of the failures I’ve seen thus far is that we know that drug addiction therapy may be effective when it is evidence-based, easily available, reasonably priced, and customizable.

With our focus on the locations that are doing things correctly, we hope to create an example that the rest of the industry and policymakers can learn from as well.
